Completes and documents initial resident assessments for all admissions and re-admissions and all required ongoing assessments including petitions to the hospital for psych residents.
Assists new residents with the orientation to the facility.
Documentation of any behaviors and incidents of residents during the shift.
Advocates for the resident during the resident’s entire stay with Grasmere Place including admission and discharge.
Researches and provides information on services to resident members in need of additional support and resources.
May be asked to participate in Care Plan Conferences.
Coordinates problem-resolution process across departments that address resident, and or staff concerns.
Discusses current residents’ plan of care.
